revision - name on the cartiglio

Good morning.

Since it is a source of discussion from me, and I have not found (mea culpa) the rule of reference that tells me this particular, I ask you.

when creating a revision, the name on the review cartiglio must be:

a. the user who created the file originally, and the auditor's name with the modification in the relative revision table;
b. the user who performs the revision, losing the name of the original author;

I would propose for "a", but I have no basis to discuss.

I usually have as design author in revision 0 in table then in the revision table is present the reviewer's name with date and description review

idem. I have the initial author of the drawing in the paper, and who approved it.
then in the revision table, the author of the revision etc..
